About Anowise Cream
Anowise Cream combines Lidocaine 1.5% w/w with Nifedipine 0.3% w/w in a topical cream formulation specifically indicated for the management of chronic anal fissures. The combination addresses the two main pain-generating mechanisms of anal fissure — local pain at the fissure site and internal anal sphincter hypertonia.
Lidocaine is an amide local anaesthetic that blocks sodium channels in cutaneous nerve endings, providing rapid pain relief within minutes of application. Nifedipine is a dihydropyridine calcium channel blocker that, when applied topically, relaxes the internal anal sphincter — improving anodermal blood flow and allowing the fissure to heal. Combined topical therapy has largely replaced surgery as first-line management of chronic anal fissures.
Anowise Cream is manufactured by Trumac Healthcare under WHO-GMP conditions and is available for monopoly-based PCD pharma franchise and third-party manufacturing — a specialty proctology SKU with strong patient compliance.
Indications
- Acute and chronic anal fissures
- Pain and discomfort during defecation in anal fissure
- Adjunct to dietary and bowel-habit management of fissure
- Symptomatic relief of haemorrhoidal pain
- Local anaesthesia before proctoscopy or perianal procedures
Mechanism of Action
Lidocaine reversibly blocks sodium channels in sensory nerve endings, preventing nociceptive impulse propagation and providing rapid local anaesthesia. Nifedipine blocks L-type calcium channels in internal anal sphincter smooth muscle, reducing sphincter tone, improving anodermal microcirculation and creating the conditions needed for fissure healing — without the systemic hypotension seen with oral therapy.
Dosage & Administration
Apply a small pea-sized amount around the anal fissure using the applicator, twice daily (morning and after evening defecation) for 4–6 weeks. Continue until the fissure heals completely. Wash hands before and after application.
Side Effects
Local irritation, mild burning at site of application. Rarely: headache, dizziness, flushing (from systemic absorption of Nifedipine). Allergic contact dermatitis to Lidocaine is uncommon but possible.
Contraindications & Precautions
Hypersensitivity to amide local anaesthetics, Nifedipine or any excipient. Severe cardiovascular disease (hypotension, cardiogenic shock). Application to broken or infected skin. Pregnancy and lactation — use only if clearly necessary. Avoid contact with eyes.
Storage
Store below 30 °C in a dry place, protected from heat and direct sunlight. Replace cap tightly after use.
