About Apixer-5 Tablet

Apixer-5 Tablet contains Apixaban 5 mg, the standard adult dose of this leading direct oral anticoagulant. The 5 mg twice-daily regimen is used for the prevention of stroke and systemic embolism in non-valvular atrial fibrillation, and for the treatment and secondary prevention of deep vein thrombosis (DVT) and pulmonary embolism (PE).

Compared with warfarin, Apixaban offers fixed dosing without routine INR monitoring, fewer drug and food interactions, faster onset and offset of effect, and — most importantly — significantly lower rates of intracranial haemorrhage. Head-to-head DOAC trials place Apixaban with the most favourable bleeding profile in its class, making it a frequent first-line choice in elderly and frail patients.

Indications

• Stroke and systemic embolism prevention in non-valvular atrial fibrillation
• Treatment of deep vein thrombosis (DVT) and pulmonary embolism (PE)
• Prevention of recurrent DVT/PE after initial treatment
• VTE prophylaxis after major orthopaedic surgery (lower dose used)

Mechanism of Action

Apixaban is a highly selective, reversible direct inhibitor of free and clot-bound Factor Xa. By preventing thrombin generation from prothrombin, it inhibits fibrin formation and thrombus growth. Apixaban does not require antithrombin for its activity (unlike heparin) and produces predictable anticoagulation at fixed oral doses.

Dosage & Administration

AF stroke prevention: 5 mg twice daily (2.5 mg twice daily if dose-reduction criteria met). DVT/PE treatment: 10 mg twice daily for 7 days, then 5 mg twice daily for at least 3 months, then 2.5 mg twice daily for extended prevention. Swallow with or without food.

Side Effects

Bleeding (most common: epistaxis, GI bleeding, haematuria, easy bruising). Anaemia, nausea. Rare: major bleeding, intracranial haemorrhage, hypersensitivity reactions. Lower intracranial bleeding risk than warfarin.

Contraindications & Precautions

Active clinically significant bleeding. Hepatic disease with coagulopathy. Concomitant strong CYP3A4 + P-gp inhibitors or inducers. Pregnancy and breastfeeding. Prosthetic heart valves. Recent intracranial/spinal surgery. Severe renal impairment (CrCl < 15 mL/min).
