About Cefom-TZ 1.125g Injection
Cefom-TZ 1.125g Injection combines Cefoperazone 1000 mg with Tazobactam 125 mg. Tazobactam is a newer beta-lactamase inhibitor with broader inhibitory activity than sulbactam — particularly against extended-spectrum beta-lactamases (ESBLs) — making this combination more useful for ESBL-producing gram-negative infections.
Cefom-TZ is positioned as an alternative to Cefoperazone-Sulbactam in patients with ESBL-producing Enterobacteriaceae or as step-down from piperacillin-tazobactam in selected cases. The combination retains cefoperazone’s anti-pseudomonal coverage with extended beta-lactamase inhibition.
Cefom-TZ is manufactured by Trumac Healthcare under WHO-GMP injectable conditions. Available for monopoly-based PCD pharma franchise, government tenders and third-party manufacturing.
Indications
- Hospital-acquired infections with ESBL-producing organisms
- Septicaemia and bacteraemia
- Complicated intra-abdominal infections
- Complicated UTI with ESBL
- Skin and soft-tissue infections with mixed flora
Mechanism of Action
Cefoperazone is a third-generation cephalosporin with strong gram-negative coverage including Pseudomonas aeruginosa. It binds penicillin-binding proteins to inhibit bacterial cell wall synthesis. Sulbactam (or tazobactam in combinations) is a beta-lactamase inhibitor that extends the spectrum to beta-lactamase-producing strains. Tazobactam is a penicillanic acid sulfone beta-lactamase inhibitor with broader activity against ESBLs (TEM, SHV, CTX-M types) than sulbactam.
Dosage & Administration
Adults: 1.125–2.25 g every 12 hours by IV/IM. Severe infections: higher doses as advised. Adjust in hepatic/renal impairment. Strictly hospital use.
Side Effects
Similar to Cefom-S: injection site reactions, GI upset, rash, disulfiram-like alcohol reaction, hypoprothrombinaemia (rare).
Contraindications & Precautions
Hypersensitivity to cephalosporins. Severe penicillin allergy. Concurrent alcohol use. Severe hepatic impairment.
Storage
