About Budemide-L Respules
Budemide-L Respules combine Budesonide 0.5 mg with Levosalbutamol — the R-enantiomer of salbutamol — in a single nebuliser respule. The combination provides rapid bronchodilation plus anti-inflammatory action in a single dose, particularly useful in acute asthma exacerbations, wheezing in children and acute bronchitis.
Levosalbutamol is preferred over racemic salbutamol because the R-enantiomer is solely responsible for bronchodilation, while the S-enantiomer (removed in Levosalbutamol) is associated with paradoxical bronchoconstriction and pro-inflammatory effects. The result is comparable bronchodilation with fewer cardiac side effects (tremor, tachycardia).

Mechanism of Action
Levosalbutamol selectively activates β2 receptors on airway smooth muscle, producing rapid bronchodilation within minutes lasting 4–6 hours, without the antagonistic action of the S-enantiomer. Budesonide binds glucocorticoid receptors and suppresses inflammatory gene expression to address the underlying airway inflammation.
Dosage & Administration
Adults: 1 respule two to three times daily by nebulisation. Children: 1 respule one to two times daily. Rinse mouth after use.
Side Effects
Tremor (less than racemic salbutamol), palpitations, headache, oral candidiasis, dysphonia, throat irritation.
Contraindications & Precautions
Hypersensitivity to either component. Tachyarrhythmias. Hypersensitivity to milk proteins (some formulations). Severe paradoxical bronchospasm.
