Mechanism of Action
Heparin binds antithrombin III causing a conformational change that accelerates its inhibition of Factor IIa (thrombin) and Factor Xa by ~1000-fold. Reversible action — short half-life (1–2 hours).
Dosage & Administration
- Loading dose: 80 IU/kg IV bolus then continuous infusion 18 IU/kg/hour.
- Adjust based on aPTT target 1.5–2.5x control.
Side Effects
- Bleeding, heparin-induced thrombocytopenia (HIT), osteoporosis with prolonged use, transaminase elevation.
Contraindications
- Active bleeding, severe thrombocytopenia, history of HIT, severe uncontrolled hypertension, recent intracranial haemorrhage.
Storage
Store between 2–8 °C. Do not freeze. Protect from light.
