Mechanism of Action
Carbimazole is a prodrug rapidly converted to methimazole, which inhibits thyroid peroxidase — the enzyme responsible for iodination and coupling of tyrosine residues on thyroglobulin. This blocks the synthesis of T4 and T3 without affecting their release. Onset of clinical effect requires 2–4 weeks for preformed hormone stores to be depleted.
Dosage & Administration
- Maintenance: 5–15 mg daily in divided doses, titrated to TSH and free T4.
- Monitor TFTs every 6–8 weeks.
- CBC and LFTs at baseline and as clinically indicated.
Side Effects
- Rash, pruritus, mild GI upset, taste disturbance.
- Rare but serious: agranulocytosis (sore throat / fever — STOP and check CBC), hepatotoxicity, vasculitis, ANCA-positive vasculitis.
Contraindications
- Hypersensitivity.
- Severe hepatic disease.
- Severe bone marrow disorders.
- Pregnancy first trimester (use propylthiouracil instead).
- Bone marrow toxicity history with thionamides.
