Mechanism of Action
Pregabalin is a structural analogue of GABA — binds alpha-2-delta subunit of voltage-gated calcium channels in CNS, reducing release of excitatory neurotransmitters (glutamate, noradrenaline, substance P). FDA-approved for neuropathic pain, fibromyalgia, partial seizures, generalised anxiety disorder. Nortriptyline is a tricyclic antidepressant blocking serotonin and noradrenaline reuptake — augments descending inhibitory pain pathways. Lower anticholinergic burden than amitriptyline.

Side Effects
Dizziness, somnolence, peripheral oedema, weight gain, blurred vision, dry mouth. Discontinuation requires gradual tapering. Anticholinergic effects.
