Mechanism of Action
Carvedilol is a non-selective beta-blocker with additional alpha-1 blockade — reduces afterload via vasodilation. One of three guideline-recommended beta-blockers for heart failure (COMET, COPERNICUS trials). Strong evidence in HFrEF.
Dosage & Administration
Adults: 3.125 mg twice daily for 2 weeks, then double every 2 weeks to target 25 mg twice daily.
Side Effects
Bradycardia, hypotension, dizziness, fatigue, fluid retention initially in HF.
Contraindications & Precautions
Severe bradycardia, AV block, decompensated heart failure, severe hepatic impairment, severe asthma.
