Mechanism of Action
Mannitol is a hexitol freely filtered at the glomerulus but not reabsorbed. By raising plasma osmolality it draws water out of cells, particularly the brain across an intact blood-brain barrier — reducing ICP. Within the tubule it retains water osmotically and promotes diuresis. Glycerin contributes its own osmotic effect and adds glucose-based energy substrate metabolised by the liver.
Dosage & Administration
- Raised ICP: 0.25–1 g/kg as IV infusion over 30–60 minutes; may be repeated 4–6 hourly
- Acute glaucoma: 1.5–2 g/kg over 30 minutes preoperatively
- Monitor serum electrolytes, osmolality, urine output
- Use only in a critical-care or operating-room setting

- Fluid and electrolyte imbalance
- Hyperkalaemia / hyponatraemia
- Pulmonary oedema if overload
- Headache, chills, fever
- Phlebitis at infusion site
Contraindications
- Severe pulmonary congestion or pulmonary oedema
- Active intracranial bleeding (not for closed cranium)
- Severe dehydration
- Anuria from acute tubular necrosis
- Hypersensitivity to mannitol or glycerin
