Mechanism of Action
Ivermectin binds glutamate-gated chloride channels in invertebrate nerve and muscle cells, causing hyperpolarisation, paralysis and death of the parasite. Mammalian cells lack glutamate-gated chloride channels in the peripheral nervous system; ivermectin also has poor penetration of the mammalian blood-brain barrier (P-glycoprotein efflux), accounting for its safety in humans.
Dosage & Administration
- Scabies: 200 mcg/kg as single dose, repeated after 7–14 days
- Adults > 65 kg typically: 1 tablet (12 mg) as single dose
- Strongyloidiasis: 200 mcg/kg daily for 2 days
- Take on empty stomach with water

- Mild GI upset
- Mazzotti-like reaction in onchocerciasis (skin pruritus, oedema)
- Mild dizziness
- Postural hypotension occasionally
- Eosinophilia
Contraindications
- Hypersensitivity to ivermectin
- Children under 15 kg
- Pregnancy 1st trimester
- Loa loa co-infection (risk of encephalopathy)
- Severe hepatic impairment
