Mechanism of Action
Ciprofloxacin inhibits bacterial DNA gyrase (topoisomerase II) and topoisomerase IV. These enzymes are essential for bacterial DNA replication, repair, recombination and transcription. By preventing supercoiling and decatenation of bacterial DNA, ciprofloxacin produces rapid bactericidal action — concentration-dependent killing with a long post-antibiotic effect.
Dosage & Administration
- Adults: 250–500 mg twice daily for 5–7 days (uncomplicated UTI may need only 3 days).
- Adjust dose in renal impairment.
- Take with adequate fluids;
- avoid simultaneous antacids, iron, calcium, dairy.
Side Effects
- Nausea, diarrhoea, headache, dizziness, abnormal taste.
- Important class effects: tendinopathy and tendon rupture (especially Achilles), QT prolongation, peripheral neuropathy, CNS effects (insomnia, anxiety).
- Photosensitivity.
- Hypoglycaemia in diabetics on sulphonylureas.
Contraindications
- Hypersensitivity to quinolones.
- Children/adolescents (cartilage toxicity in growth plates) — except specific indications.
- Pregnancy and lactation.
- Tendon disorder history with quinolones.
- Concomitant tizanidine (severe interaction).
- Caution with QT-prolonging drugs, antidiabetics, and in epilepsy.
