Mechanism of Action
Cefixime binds penicillin-binding proteins (PBP-3) on gram-negative bacteria, inhibiting the final transpeptidation step of peptidoglycan synthesis. The result is bacterial cell wall destabilisation and osmotic lysis. Cefixime is resistant to many beta-lactamases produced by gram-negative organisms — distinguishing it from earlier cephalosporins. Lactobacillus produces lactic acid in the colon, suppressing pathogenic C. difficile growth and reducing AAD risk.
Dosage & Administration
- Adults: 1 tablet twice daily for 5–7 days;
- longer in complicated infections.
Side Effects
- Reduced incidence of diarrhoea compared with plain cefixime.
- Otherwise: nausea, mild rash, hypersensitivity.
- Rare: pseudomembranous colitis still possible.
Contraindications
- Hypersensitivity to cephalosporins.
- Severe penicillin allergy (cross-reactivity ~10%).
- Renal impairment requires dose adjustment.
- History of GI disease (caution).
- Severe immunosuppression (caution with live probiotics).
