- Adjunctive therapy of focal-onset (partial) seizures with or without secondary generalisation

Mechanism of Action
Brivaracetam is a selective, high-affinity ligand of synaptic vesicle protein SV2A in the presynaptic terminal. Binding SV2A modulates neurotransmitter release in epileptic foci, reducing the abnormal hypersynchronous firing that underlies focal seizures. Unlike older antiepileptics, it does not act primarily through sodium-channel, GABA or glutamate receptors.
Dosage & Administration
- Adults: starting dose 50 mg twice daily;
- may be adjusted to 25–100 mg twice daily based on response and tolerability.
- Dose adjustment in hepatic impairment.
- May be taken with or without food.
- Can be initiated or discontinued without titration (advantage over many AEDs).
Side Effects
- Drowsiness, dizziness, fatigue, headache, nausea, vomiting.
- Less commonly: behavioural disturbances (less than levetiracetam), bronchospasm, angioedema (rare).
Contraindications
- Hypersensitivity to brivaracetam, levetiracetam or any pyrrolidone derivative.
- Caution in hepatic impairment, psychiatric history, and in patients on sedating medications.
- Pregnancy — limited data;
- use only if essential.
