Therapeutic Indications
- Prophylaxis of VTE after hip or knee replacement surgery
- Extended VTE prophylaxis (after initial 6 months of treatment)
- Stroke and systemic embolism prevention in non-valvular AF (dose-reduction criteria: age ≥80, weight ≤60 kg, creatinine ≥1.5 mg/dL — any two of these)
- Secondary prevention of recurrent VTE
Mechanism of Action
Apixaban is a highly selective, reversible direct inhibitor of free and clot-bound coagulation Factor Xa. By inhibiting Factor Xa, Apixaban prevents the conversion of prothrombin to thrombin, suppresses fibrin formation, and reduces thrombus growth and propagation — without directly affecting platelet aggregation.
Dosage & Administration
- VTE prophylaxis after hip/knee replacement: 2.5 mg twice daily for 12–35 days.
- Stroke prevention in AF (dose-reduction criteria met): 2.5 mg twice daily.
- Extended VTE prevention: 2.5 mg twice daily.
- Swallow with or without food.
Side Effects
- Bleeding (most common: epistaxis, GI bleeding, haematuria, easy bruising).
- Anaemia, nausea.
- Rare: major bleeding, intracranial haemorrhage, hypersensitivity reactions.
- Compared with warfarin: lower intracranial bleeding rate.
Contraindications
- Active clinically significant bleeding.
- Hepatic disease with coagulopathy.
- Concomitant strong CYP3A4 + P-gp inhibitors (ketoconazole, ritonavir) or inducers (rifampicin).
- Pregnancy and breastfeeding.
- Prosthetic heart valves.
- Recent intracranial/spinal surgery.
- Severe renal impairment (CrCl < 15 mL/min).
