Mechanism of Action
Amikacin binds irreversibly to the 30S bacterial ribosomal subunit, causing mRNA misreading and inhibition of protein synthesis. The resulting non-functional proteins damage bacterial cell membranes, producing rapid bactericidal effect. Activity is concentration-dependent with a long post-antibiotic effect supporting once-daily dosing.
Dosage & Administration
- Adults: 15 mg/kg/day, typically given as a single daily dose by IM injection or slow IV infusion.
- Dose adjustment required in renal impairment based on serum levels.
- Treatment duration typically 7–10 days.
Side Effects
- Nephrotoxicity, ototoxicity (cochlear and vestibular), neuromuscular blockade.
- Local injection site pain.
- Less commonly: hypersensitivity, rash, transient hepatic enzyme elevation.
Contraindications
- Hypersensitivity to aminoglycosides.
- Concomitant nephrotoxic/ototoxic drugs without monitoring.
- Pre-existing 8th cranial nerve damage.
- Myasthenia gravis.
- Pregnancy (Category D).
- Therapeutic drug monitoring strongly advised in renal impairment.
