Mechanism of Action
Alfuzosin selectively blocks alpha-1A/1D adrenergic receptors in the prostate, bladder neck and urethra, relaxing smooth muscle tone for immediate symptom relief. Dutasteride inhibits both type 1 and type 2 isoforms of 5-alpha-reductase, dramatically reducing serum and intra-prostatic DHT levels, which over months produces apoptosis of androgen-dependent prostatic tissue and a 20–25% reduction in prostate volume.
Dosage & Administration
- Adults: one tablet once daily, taken at the same time each day with or without food.
- Swallow whole.
- Long-term therapy required for full Dutasteride benefit (6–12 months for prostate-volume reduction).
Side Effects
- Alfuzosin: dizziness, postural hypotension, headache, fatigue.
- Dutasteride: decreased libido, erectile dysfunction, ejaculation disorders, gynaecomastia, breast tenderness.
- Combined regimen may increase ejaculatory dysfunction frequency.
Contraindications
- Hypersensitivity to either component or other 5-alpha-reductase inhibitors (finasteride).
- Women — particularly pregnant or potentially pregnant women — must not handle leaking capsules (teratogenicity risk).
- Severe hepatic impairment.
- Concomitant potent CYP3A4 inhibitors.
- Not for use in children.
- Inform ophthalmologist before cataract surgery (IFIS risk).
